Schedule: Day Shift | 5x8 Hours | 8:00 AM – 4:30 PM
Job Summary
"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager for an acute care hospital assignment in Apple Valley, CA. The ideal candidate must have strong hospital-based case management and discharge planning experience with the ability to work independently in a fast-paced healthcare environment.
Required Qualifications
- Active California RN License required
- Minimum 2 years of Acute Care Case Management / Discharge Planning experience required
- Hospital experience required
- First-time travelers will not be considered
- Strong communication and coordination skills
- Ability to manage high patient caseloads effectively
Key Responsibilities
- Coordinate patient discharge planning and continuity of care
- Collaborate with physicians, nursing staff, and interdisciplinary teams
- Review patient care plans and utilization needs
- Arrange post-discharge services and resources
- Maintain accurate patient documentation and case records
- Support patients and families throughout hospitalization and discharge process
- Ensure compliance with hospital policies and care management standards
