Job Title: Direct Care Specialist – Day Activity Support
Position Overview
Array of Brighter Beginnings, Inc. is seeking dynamic and compassionate individuals to join our team as Direct Care Specialists in Day Activity Support. This role operates within the social work and human services field and works directly with individuals diagnosed with Intellectual and Developmental Disabilities (IDD) and/or Mental Health (MH) challenges. Day Activity Support specialists deliver care aligned with Long-Term Community Support (LTCS), 1915(i) medicaid waivers, state-funded initiatives, Innovations, and other meaningful day services, providing structured community engagement for up to 6 hours per day, 5 days a week.
Through outreach, community integration, and direct collaboration, the specialist helps individuals achieve their specific personal milestones as outlined in their Person-Centered Plan (PCP). This position is ideal for individuals who value a flexible work schedule, enjoy organizing and participating in recreational activities, and want to make a tangible difference in someone’s daily life. We prioritize matching individuals with specialists based on compatible interests and personalities to ensure a rewarding experience for both.
Key Responsibilities
- Community Integration & Recreation: Facilitate and actively participate in physical, recreational, social, and educational activities. Serve as a positive role model while helping individuals navigate public and community spaces.
- Skill Development & Independence: Support individuals in their homes, day programs, and the community to function more safely and independently. Assist with daily life skills, basic needs, and behavior management training.
- Goal Advocacy: Help individuals identify and pursue educational goals, seek out employment opportunities where applicable, and manage schedules for medical or psychiatric appointments.
- Resource Mapping: Identify social, economic, recreational, and educational resources within the local community that align with the individual’s unique needs.
- Care Team Collaboration: Monitor the individual’s overall well-being. Work closely with the Brighter Beginnings case management team to identify potential challenges and provide valuable clinical input to counselors and professionals regarding individual program plans.
- Administrative Tasks: Maintain accurate, timely documentation using basic laptop/computer skills. This includes writing daily progress notes, completing monthly documentation, and filing incident reports as required.
- Additional service opportunities may be available based on organizational needs and provider qualifications, including Respite Care and Alternative Family Living (AFL). Qualifications & Expectations
- Adaptability & Flexibility: Work hours are highly flexible and established collaboratively based on your availability and the individual’s specific daytime schedule.
- Professional Growth: Experience in the IDD/MH field is a plus, but not required. Comprehensive industry training is provided for passionate candidates who are new to the field.
- Collaboration: Ability to attend all scheduled professional trainings and interdisciplinary team meetings.
