Care Team Specialist (CTS)
Reports To: Director of Client Services or Care Manager
Position Summary
The Care Team Specialist (CTS) is a field-based leadership role responsible for supporting clients, mentoring caregivers, and ensuring high-quality service delivery. The CTS serves as a role model by demonstrating professionalism, adherence to company policies, and a positive, team-oriented attitude.
This position is primarily hands-on and requires flexibility to assist with client care, caregiver training, emergency staffing needs, client visits, and quality assurance activities.
Qualifications
Current Certified Nursing Assistant (CNA) certification preferred.
Combination of education and experience totaling at least three years in healthcare, home care, or a related field preferred.
Current on all certifications, training requirements, and continuing education required by Right at Home and the State of Kansas.
Experience providing personal care services in a home care or facility setting preferred.
Key Responsibilities
Client Care & Service Delivery
Ensure services are delivered according to each client's care plan.
Provide direct client care as needed.
Assist with care starts, client visits, reassessments, and quality assurance visits.
Fill open shifts due to call-offs, emergencies, and urgent staffing needs.
Communicate with client concerns and changes in condition to the appropriate office staff/caregivers and make updates to the plan of care accordingly.
Caregiver Training & Support
Provide hands-on training and mentoring for caregivers in the home and in the office training room, Betty’s Place.
Conduct caregiver support visits and coaching in the field.
Train caregivers on safe transfer techniques and the proper use of mobility equipment.
Maintain regular communication with new caregivers through phone, email, and in-person visits.
Support caregiver retention efforts and participate in the retention team.
Leadership & Team Support
Follow and promote company policies and procedures.
Foster a positive, respectful, and collaborative team environment.
Provide recommendations to improve training, service delivery, and caregiver support.
Maintain professional boundaries with clients and caregivers.
Represent Right at Home professionally at all times.
On-Call & Staffing Support
Participate in an agreed-upon supervisor on-call rotation.
Assist with emergency staffing situations, caregiver call-offs, and urgent client needs.
Work closely with the Scheduling Manager and Client Services team to ensure continuity of care.
Documentation & Compliance
Complete all required documentation accurately and on time.
Conduct client and caregiver quality audits by phone, email, and in person.
Maintain confidentiality and comply with HIPAA requirements.
Follow all Right at Home policies and procedures.

Requirements
Up to three years of CNA, Home Care Aide, or Caregiving experience preferred.
Knowledge of safe transfer techniques and mobility equipment including Hoyer lifts, sit-to-stand lifts, wheelchairs, slide boards, transfer poles, and gait belts.
Strong verbal and written communication skills.
Self-motivated, organized, and able to work independently.
Maintain a professional appearance and conduct consistent with company standards.
Valid driver's license, reliable transportation, and ability to travel throughout the Right at Home Kansas City area. Mileage reimbursement is provided.
Smartphone capable of using company scheduling, communication, and documentation systems.
Attend all required meetings, training, and continuing education programs.
Additional Responsibilities
The Care Team Specialist may be assigned additional duties as needed to support clients, caregivers, and business operations.
