The Manitowoc County Human Services Department is seeking a compassionate, organized, and motivated professional to join our Family Resiliency Unit as a Children’s Long-Term Support (CLTS) Waiver Specialist.
The Children’s Long-Term Support (CLTS) Program helps children with disabilities and their families access the supports and services they need to thrive at home, in school, and in their communities. In this rewarding role, you will work directly with families to coordinate care, build meaningful service plans, and support positive outcomes for children across Manitowoc County.
As a CLTS Waiver Specialist, you will:

Provide case management through regular contact with families and service providers

Participate in collaborative meetings and attend appointments as needed
-
Maintain timely, accurate documentation and organized case records
-
Process referrals and gather information from collateral sources
-
Administer functional screens to determine eligibility and level of care needs
-
Assist families in identifying outcome-based goals and developing individualized service plans

Coordinate services that empower families to safely care for their children at home

A Bachelor's degree in human services related field and Wisconsin driver's license is required.
