Job Title: Patient Advocate in a nursing home
Position Summary
The Patient Advocate serves as a key liaison between patients, families, and the facility, ensuring concerns are addressed promptly, rights are upheld, and care experiences are positive. This role focuses on communication, problem-solving, and patient satisfaction while supporting regulatory compliance and facility standards.
Must work every other weekend
Key Responsibilities
- Act as the primary point of contact for patient and family concerns, questions, and grievances
- Advocate for patient rights, dignity, and informed decision-making
- Investigate complaints, document findings, and coordinate resolutions in a timely manner
- Collaborate with Admissions, Nursing, Social Work, and clinical teams to resolve issues
- Educate patients and families on facility policies, and available resources
- Assist patients and families with navigating the internal healthcare system
- Escalate issues to respective departments and maintain accurate records of grievances and outcomes
- Ensure compliance with New York State Department of Health and federal regulations
- Participate in quality improvement initiatives and patient satisfaction efforts
- Support surveys, audits, and inspections as needed
- Complete required Admission Agreement paperwork with patient / family
Qualifications
- Bachelor’s degree preferred (Social Work, Healthcare Administration, Psychology, or related field)
- Minimum 1–3 years of experience in healthcare, patient advocacy, social services, or customer relations
- Experience in a skilled nursing or sub-acute setting strongly preferred
- Knowledge of patient rights, HIPAA, and NYS healthcare regulations
- Strong interpersonal, communication, and conflict-resolution skills
- Ability to handle sensitive situations with professionalism and empathy
- Excellent documentation and organizational skills
- Bilingual Spanish required
