Qualifications
- Home health experience preferred
- OASIS experience desired. We can provide training.
- Current Physical Therapist (PT) license in the state of practice
- Valid driver’s license and reliable transportation
- Current CPR certification
- Strong clinical judgment and time-management skills
- Ability to work independently in the home setting
- New graduates welcome with appropriate support
Key Responsibilities
- Perform comprehensive physical therapy evaluations in patient homes
- Develop, implement, and update individualized plans of care in collaboration with physicians
- Provide skilled interventions including therapeutic exercise, gait training, balance training, functional mobility, and pain management
- Reassess patients regularly and adjust treatment plans as needed
- Educate patients and caregivers on home exercise programs, fall prevention, and safety
- Supervise Physical Therapist Assistants (PTAs) as applicable and per state regulations
- Communicate effectively with physicians, nurses, and interdisciplinary team members
- Complete accurate and timely clinical documentation in the EMR
- Comply with Medicare, Medicaid, and agency policies and procedures
Job Types: Part-time, PRN, Contract, Per diem, Temporary
Pay: $60.00 - $80.00 per hour
Expected hours: 1.0 – 20.0 per week
