Key Responsibilities:
- Assess resident health conditions and contribute to the development of resident care plans.
- Administer medications and treatments as prescribed.
- Monitor residents’ progress and respond to changes in their condition.
- Provide education and health coaching to residents and their families regarding health management.
- Communicate effectively with residents, families, and interdisciplinary teams.
- Ensure compliance with all safety, infection control, and regulatory guidelines.
- Ensures that a safe and effective resident care environment is maintained.
- Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team.
- Safely administers and documents all forms of medication administration.
- Assesses, plans, implements and evaluates care based on age-specific components.
- Identify and assess resource utilization and safety issues, taking appropriate action.
- Aligns nursing/clinical objectives with organizational strategic outcomes, participating in strategy planning and execution.
- Treats all residents, visitors, and staff with courtesy and dignity.
Qualifications:
- Current and valid licensure as a Registered Nurse in the State of New York.
- Ability to work effectively within a multidisciplinary team.
