Gotham Companies is seeking skilled and compassionate UAS Nurse Assessors to conduct person-centered assessments for individuals in need of Long-Term Care Services across New York State. Your Impactful Role Includes:
- UAS Assessments: Conduct initial and reassessments in line with NYS standards, accurately determining eligibility for long-term care services.
- Regulatory Compliance: Ensure every assessment adheres to NYS guidelines, including requirements for Medicaid/Medicare and Managed Long-Term Care (MLTC) plans.
- Accurate Documentation: Diligently record findings, concerns, and other key information in the UAS electronic system to ensure seamless follow-up.
- Client Advocacy & Planning: Provide valuable information and care options to clients and families, supporting informed decisions around long-term care.
- Collaboration: Work closely with interdisciplinary teams to deliver cohesive, quality care that benefits each client’s health and well-being.
- Quality & Regulatory Standards: Stay on top of evolving UAS protocols and best practices to ensure all assessments meet NYS Department of Health standards.
- On-Site Visits: Conduct in-person evaluations in clients' homes or facilities, meeting NYS requirements for thorough and personalized assessment.
Qualifications Needed:
- Active NYS RN License: Unencumbered New York State RN license with valid NYS ID.
- UAS Certification: Active UAS User ID number required.
- Experience: At least 1 year of UAS or home care assessment experience, with familiarity in MLTC and Medicaid/Medicare plans.
- Skills: Strong clinical assessment, critical thinking, and documentation abilities.
- Tech Proficiency: Prior experience with electronic health records (UAS evaluation forms) and QA tools like Lenavi & QAS is a plus.
- Communication: Excellent interpersonal skills for effective interaction with clients, families, and interdisciplinary teams.
- Commitment to Excellence: A strong grasp of best practices in UAS assessments and dedication to maintaining current certifications and standards.
Work Schedule:
- Flexible, Fee-for-Service Model
Compensation: Based on the contract and type of visit
Job Types: Part-time, PRN, Per diem
Pay: $1,500.00 - $2,500.00 per week
