Licensed Social Worker - BSW/Case Manager
Autumnleaf Group is seeking a full-time contract Licensed Case Manager to provide comprehensive case management for the elderly and physically disabled in Washington, DC and Virginia. Home visits are grouped by region. Work will be performed in multiple locations (on the road) and needs 50% travel - reliable transportation is required and mileage is reimbursed.
Administrative days are remote with monthly on-site in DC, reporting typically scheduled in advance and with 8-10 days per month of in-person home visits in DC. Visits are 100% in-person face to face. Typical start time: removed
What you will do:
- Engage clients face to face to complete comprehensive case management.
- Accomplish clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; and facilitating interdisciplinary approaches.
- Identify and assess available services to meet clients’ unique social and emotional needs.
- Attend required onsite/virtual meetings and evaluations to communicate clients’ progress.
- Operate different types of computer systems and utilize various software applications to document all care management/coordination activity, participate in meetings, and perform other tasks.
- Continuously update job knowledge and technical skills by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
Required Education Level and Work Experience:
- At least bachelors degree in Social Work with active social work license and preferred 2 years of work experience. Internship experience counts.
- Current District of Columbia Social Work Licensure (LSWA or higher) is required.
- Ability to manage 45 cases (full-time).
- Social Work experience required.
- Long term care case management experience required.
- Experience working with managed care and Medicare/Medicaid patients.
- Excellent counseling skills and a resourceful approach to problem solving.
- Exceptional documentation skills.
- Exceptional planning and organizational ability.
- Strong collaboration skills.
- Available to work remotely productively with periodic on-site in the DC office and to conduct home visits in DC 8-10 days monthly.
- DC rules require all healthcare professionals licensed in DC to be fully vaccinated.
Salary starting at $67,000 annually, commensurate with experience/qualifications.
