Position Summary
The Licensed Practical Nurse (LPN) is responsible for providing direct nursing support and clinical services to clients in accordance with physician orders, individualized care plans, company policies, and all applicable state and federal regulations. The LPN supports quality patient care, accurate documentation, medication administration, communication with the care team, and compliance with all clinical and safety standards in home care, residential, and/or skilled settings.
Essential Duties and Responsibilities
Provide direct nursing care and clinical support to clients in accordance with physician orders and individualized care plans. Administer medications, treatments, and nursing interventions as ordered and document appropriately. Monitor, observe, and report changes in client condition, behavior, or status to the appropriate clinical supervisor. Assist with client assessments, admissions, recertifications, resumptions of care, and other clinical documentation as assigned. Accurately complete nursing notes, medication records, incident reports, supervisory notes, and other required documentation in a timely manner. Maintain compliance with all applicable company policies, state regulations, EVV requirements (if applicable), and payer standards. Collaborate with RNs, Nurse Supervisors, House Managers, DSPs, caregivers, physicians, and interdisciplinary team members to ensure continuity of care. Respond promptly to incidents, changes in condition, hospitalizations, and urgent client needs according to company protocol. Support staff training and reinforce proper care procedures, documentation standards, and client safety practices when needed. Participate in audits, meetings, chart reviews, case conferences, and quality improvement activities as directed. Ensure confidentiality and compliance with HIPAA and all client privacy standards. Perform other clinical duties as assigned by management.
Qualifications
Active Licensed Practical Nurse (LPN) license in good standing in the Commonwealth of Pennsylvania. Current CPR certification (required or preferred based on assignment). Minimum of one (1) year of nursing experience preferred; home care, home health, residential, behavioral health, or long-term care experience strongly preferred. Strong clinical judgment, communication, organizational, and documentation skills. Ability to work independently and collaboratively within a multidisciplinary team. Ability to complete all required clearances, background checks, and onboarding requirements. Reliable transportation and ability to travel to assigned client or program locations as needed.
Pay: $35.00 per hour
