Position Summary
The Physical Therapist is responsible for evaluating patients, developing individualized plans of care, and providing evidence-based physical therapy services to improve mobility, function, and overall quality of life. The Physical Therapist continuously monitors patient progress, modifies treatment plans as needed, and collaborates with physicians and the interdisciplinary healthcare team to ensure the best possible patient outcomes. This role also includes patient and caregiver education, participation in discharge planning, and mentoring physical therapy students and assistants.

Essential Responsibilities
- Perform comprehensive physical therapy evaluations for patients across multiple age groups.
- Develop individualized treatment plans and establish measurable rehabilitation goals.
- Implement therapeutic interventions designed to restore mobility, strength, balance, and functional independence.
- Monitor patient progress and revise treatment plans as appropriate according to departmental policies.
- Complete accurate and timely documentation for every patient visit.
- Participate in interdisciplinary team meetings, care conferences, and discharge planning.
- Educate and counsel patients, families, caregivers, coaches, employers, and healthcare providers regarding treatment plans and home exercise programs.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care.
- Supervise Physical Therapist Assistants (PTAs) and Physical Therapy student interns.
- Complete a minimum of 16 hours of continuing education annually.
- Maintain compliance with departmental policies, professional standards, and regulatory requirements.

The Physical Therapist may provide evaluation and treatment for patients across all age groups, including:
- Neonatal
- Pediatric
- Adolescent
- Adult
- Geriatric
Education & Experience RequirementsEducation
Candidates must possess one of the following:
- Bachelor of Science in Physical Therapy
- Master's Degree in Physical Therapy
- Doctor of Physical Therapy (DPT)
- Certificate in Physical Therapy
Experience
- Previous clinical physical therapy experience in an acute care setting is preferred.
- Ability to independently evaluate, treat, and manage a diverse patient population.
- Strong clinical reasoning, communication, and interdisciplinary collaboration skills.
Licensure & Certifications
Required:
- Current Wisconsin Physical Therapist License
- Current Illinois Physical Therapist License
- American Heart Association (AHA) Basic Life Support (BLS) Certification
- Valid Driver's License

Pay: $52,000.00 - $79,000.00 per year
