Seasons Change Home Health Care & A Change of Seasons Hospice is seeking a compassionate, dependable, and motivated Master Social Worker (MSW) to provide exceptional psychosocial support to patients and families throughout Huron, Tuscola, and Sanilac Counties. This position offers flexibility while allowing you to make a meaningful difference in the lives of individuals receiving home health and hospice services.
Position Summary
The Master Social Worker is responsible for assessing the psychosocial, emotional, environmental, and financial needs of patients and their families. Working as a valued member of our interdisciplinary team, the MSW helps connect patients with community resources, provides counseling and support, assists with care planning, and advocates for patient well-being throughout the continuum of care.
Responsibilities
- Complete comprehensive psychosocial assessments in patients' homes.
- Identify emotional, social, financial, and environmental barriers that may impact health outcomes.
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team.
- Provide supportive counseling, crisis intervention, education, and emotional support to patients and caregivers.
- Assist patients and families in accessing appropriate community resources, financial assistance programs, advance care planning resources, and other supportive services.
- Coordinate care with physicians, nurses, therapists, hospice staff, and community agencies.
- Participate in interdisciplinary team meetings, case conferences, and care coordination activities.
- Maintain accurate, timely, and regulatory-compliant documentation within the patient's medical record.
- Assist with discharge planning and transitions of care as appropriate.
- Participate in quality improvement initiatives, staff education, and agency in-service programs.
- Maintain patient confidentiality and uphold all agency, Medicare, Medicaid, CHAP, and HIPAA standards.
Qualifications
- Current Michigan licensure as a Licensed Master Social Worker (LMSW), as permitted for the services provided.
- Minimum of two years of social work experience preferred.
- Home Health or Hospice experience is preferred but not required.
- Knowledge of community resources and available support services.
- Excellent communication, interpersonal, organizational, and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Strong assessment, critical thinking, and problem-solving abilities.
- Reliable transportation, valid Michigan driver's license, and current automobile insurance.
