POSITION SUMMARY: The role of the HOPE Community Family Care Manager is to utilize agency and community resources to offer residents the opportunity to achieve lasting recovery from homelessness. This position promotes client achievement of self-sufficiency by providing concrete pathways in reaching housing, health, and financial outcomes. In addition, this position addresses individual needs for transportation, employment, medical/mental health/recovery issues and assists clients through the application processes. This position works collaboratively with various community agencies to secure additional resources and advocating against systemic barriers to homelessness.
This position aides’ direct staff, HOPE General Manager, and executive staff in a way that fosters health, mutual respect, clear communication, and positive self regard.
QUALIFICATIONS:
- Bachelor’s degree required (social work, psychology, or a counseling profession)
- Two years of human services experience, preferably in a shelter or similar environment.
- Demonstrate capacity in providing direct care services, preferably case management, to homeless and/or marginalized or underserved populations.
- Experience with crisis intervention and management.
- Commitment to fostering professional working relationships with colleagues, residents and volunteers.
- First Aid and CPR certifications (within 30 days of hire).
DUTIES AND RESPONSIBILITIES:

Provide case management with residents, including but not limited to ongoing assessment and goal setting, crisis intervention, referrals to resources, directing and supporting as needed to help residents achieve self-sufficiency.
- Collect information to identify a resident’s housing and service needs with the intent to resolve that participant’s immediate housing crisis. In this phase, the case manager will seek information necessary to refine, clarify, and verify a resident’s housing and homeless history, barriers, goals, and preferences. This will be documented in the Needs Assessment within 14 days of their admission.
- Developing Individualized Housing Plans with residents based on the Needs Assessment within 14 days of their admission.
- Provide a minimum of biweekly engagement and monitor housing goals set with the resident. Engagement is ongoing until the household has transitioned to permanent housing.
- Increase residents’ awareness of available services applicable to individual needs and assist in accessing resources in a way which encourages personal responsibility and self-sufficiency.
- Maintain all charts and records according to NASW standards, including use of the Homeless Management Information System.
- Develop mutually beneficial working relationships with Resident Care Specialists that support residents in reaching housing, health, and financial goals.
SUPERVISORY RESPONSIBILITIES:
- Provide task supervision to Social Work interns.
- Provide MSW supervision to Social Work interns if the individual has an MSW.
SKILLS REQUIRED:
- Excellent interpersonal and customer service skills.
- Effective written and verbal communication skills.
- Strong computer literacy skills (Microsoft Word, Office, Excel and Internet).
- Ability to manage and de-escalate crises.
- Knowledge of local resources and services for families.
- Cultural competence and sensitivity.

- Physically able to work in a fast-paced multitasked environment.
- Work various shifts as required to ensure:
- Care management and client services are adequately administered and accessible.
- Adequate training and coaching of Resident Care Specialist and Interns.
- Includes on-call and weekend consultation.
