Covenant Post Acute is a 121-bed community that offers exceptional skilled nursing care for seniors recovering from surgery, rehabilitating from a specific injury or trauma, or those in need of high acuity nursing care. Essential Duties:
· Assist CNA’s in the performance of their job requirements and assume caregiver activities when needed.
- Assist both CNA’s and residents in performing activities of daily living, including; shaving, dressing, oral hygiene, toileting, bathing, incontinence care, feeding, nail care, transferring in and out of beds/wheelchairs, and the setting and clearing of tables at meal time.
- Work towards maintaining each resident’s self-respect, personal dignity, personal safety and confidentiality.
- Display tact and friendliness when dealing with residents, families, visitors and co-workers.
- Assist new residents with arranging personal items and help familiarize residents and their family members with their new surroundings.
- Encourage residents to participate in daily activities such as bed making, laundry, and setting tables.
- Communicate with community healthcare professionals, including physicians, discharge planners, nurses and state agencies on the care needs of all residents at the property.
- Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by physician.
- Conduct thorough reporting of pertinent information regarding resident care to oncoming shift. Complete documentation in and encourage use of the Communication Log on each shift.
- Participate in end-of-shift report to ensure all needed information is communicated.
- Coordinate incidental medical and dental care for residents.
- Assist the Director of Nursing with physician orders and pharmacy delivery of medications. Organize central storage and delivery of medications for each shift.
- Observe residents and collect data pertinent to resident care. Communicate all relevant resident information with physicians and other healthcare professionals as needed.
- Ensure proper documentation regarding change in condition and incident reporting for both residents and staff. Ensure all paperwork is completed and that proper documentation is maintained in the medical record.

- Assist Activity Department staff in carrying out planned activities.
- Attend all scheduled in-service classes.
- As requested, assist the Director of Nursing with orienting new staff to the residence.
- Provide support to community’s marketing efforts through positive/friendly interaction during all property tours and telephone contact with the public, and as directed by a supervisor.
- Maintain a safe and secure environment for all staff, residents and guests, following established safety standards. Perform daily and nightly rounds of the property. Notify maintenance staff of all needed repairs.
- Encourage teamwork through cooperative interactions with co-workers and other departments.
- Support a positive and professional image through actions and dress.
- Deliver medication to residents and assist with treatments per physician’s orders.
- Prepare all resident medications for delivery in a timely manner.
- Verify that resident’s medications are ingested or applied per physician’s orders. Document any refusals or errors and give detailed explanation. Notify Director of Nursing or Executive Director.
- Ensure medication carts are organized and cleaned, that carts are locked and lock is functioning properly, and appropriately stocked.
- Responsible for coordinating medication management with resident, family, physician, and pharmacy.
- Order medications as needed and directed by Director of Nursing.
- Ensure proper documentation of medication changes, new orders, and refills ordered.
- For all new residents, verify medications and dosage with physician, coordinate medication management with pharmacy, and prepare new resident paperwork.
- Maintain accurate records and complete documentation as directed by the Director of Nursing.
- As directed, assist Director of Nursing with narcotic counts and report any discrepancies immediately to Director of Nursing or Executive Director.
- Assist Director of Nursing with management and ordering of supplies.
- As directed by Director of Nursing assist with communication with residents, families, physicians, healthcare agencies, and pharmacies.
- Update 24 hour communication log and notify Director of Nursing or Executive Director of any medication changes or resident change of condition.
- Provide support to CNA’s as needed.
- Maintain current knowledge of community policies, programs, services provided, and other information as directed. Understand basic medical terminology and abbreviations used.
- Demonstrate ability to follow infection control and emergency procedures.
- Assure confidentiality in all matters, including but not limited to employee and resident information.
- Participate in daily management meetings to communicate key issues within the community.
- Maintain compliance with all Federal (Title 22) and state regulations and company policies and procedures applicable to job duties.
- Maintain a safe and secure environment for all staff, residents and guests, following established safety standards.
- Encourage teamwork among all departments and assist with conflict resolution.
- Present positive and professional image.
- Per State of California regulations, as a “mandated reporter” you must report all known or suspected incidents of resident abuse of dependent adults or elders.
- Performs other duties as assigned and consistent with the success of the position.
