DUTIES:
- Collaborates with medical providers, care managers, and social service agencies to deliver short-term care coordination and connect clients to resources.
- Develops, implements, evaluates, and adjusts individualized care plans.
- Assesses high-risk clients for immediate safety or intervention needs and provides provisional support until ongoing services are established.
- Serves as a Public Health Social Worker, linking clients to health and social programs, including insurance, healthcare, and community benefits.
- Supports access to services such as establishing a medical home, securing coverage, and addressing barriers to care.
- Connects clients with community resources for housing, food, clothing, prenatal education, parenting support, and mental health services.
- Explains program purposes and their impact on client wellbeing.
- Advocates for clients during calls and visits to reduce cultural and socioeconomic barriers and ensure needs are effectively communicated and met.
- Delivers educational sessions on parenting, emotional regulation, sexual health, and related topics.
- Maintains accurate documentation of client interactions, manages comprehensive case files, and completes monthly reports.

EDUCATION AND EXPERIENCE:
- Master’s degree in social work and a minimum of one year working with individuals or families in a community-based organizational setting; OR
- Bachelor’s degree in social work and a minimum of three years of experience working with individuals or families in a community-based organizational setting.
SPECIAL REQUIREMENTS:
- Must complete State of Michigan MI Bridges Community Referral Partner Training within six months of hire.
- Must possess and maintain a valid driver’s license with access to reliable transportation throughout the course of employment.
- Duties will require the ability to sit, walk, reach with hands and arms and occasionally lift 25-50 pounds.
- In times of a public health emergency, may be required to report for specialized assigned duties inside or outside of Berrien County.
REQUIRED KNOWLEDGE AND SKILLS:
- Experience assessing and supporting clients with suicidal ideation, depression, perinatal mood disorders, domestic violence, sex trafficking, and other high-risk safety concerns.
- Background in medical settings, including the State of Michigan MI Bridges Benefit Enrollment program and the Healthy Michigan Plan.
- Strong interpersonal skills with the ability to engage diverse populations professionally and empathetically during crisis situations.
- Committed to expanding knowledge of community resources and understanding barriers to care.
- Communicates effectively with healthcare professionals and care coordinators.
- Demonstrates strong project coordination and organizational skills, with the ability to collaborate effectively in team environments.
- Knowledge of structural influences on health, including the intersections of race, education, and income.
- Familiar with multi-system outreach programs related to healthcare delivery and services.
- Bilingual, with Spanish proficiency preferred.
