Job Summary
Under the direction of the Outreach Manager or District Director, the Certified HIV Tester/Counselor and Linkage Coordinator/Peer Navigator will carry out program activities to meet contractual agreements set by Midway Specialty Care Center on its behalf and for clients. The Tester provides HIV, STD, and HCV risk reduction counseling, HIV Rapid testing, client recruitment and targeted social media outreach to priority individuals, providing education on consistent and proper use of PrEP/condoms, and facilitating the timely entry to treatment (HIV/HCV/STD) including PEP. The Tester will also effectively engage, retain in quality medical care HIV-positive/High-risk Negative individuals, and implement client-centered service plans developed for self-efficacy, HIV Prevention, and viral suppression
Duties and Responsibilities:
- Responsible for conducting testing outside the office and targeted community outreach and engagement, risk reduction education, referrals, and case documentation including appropriate follow ups based on acuity levels.
- Complete required data collection, maintain consistent follow ups, monthly reporting, and attend all meetings associated with the navigation and support programs.
- Coordinate care with Outreach Coordinator, PrEP/PEP Navigators, Medical Case Managers, and other multidisciplinary teams.
- Provide patient-escort assistance whereby the Tester accompanies patients to their medical visits.
- Other duties as assigned.
Required Skills & Abilities
- Applicant must be an HIV Certified Tester in the State of Florida-- 500/501 Certified (or have the ability to become certified within 90 days of employment.
- Must posses a valid Florida driver’s license and maintain an acceptable driving record, as driving is required for this role and may include the use of a company vehicle.
- Experience linking at-risk/vulnerable populations preferred.
- Multi-lingual (Spanish and/or Haitian Creole); proficient in English and Spanish with proven excellent written and verbal skills.
- Sensitivity to the needs of diverse cultures and populations, and the issues that impact HCV/HIV-positive, High-risk Negative individuals and their families.
- Ability to work weekends, evenings, and follow a flexible schedule.

- Conduct comprehensive assessments on clients’ needs for primary care treatment and/or supportive services, early interventions such as PrEP/PEP, peer-based counseling, and health education in alignment with clients’ health literacy levels and language of preference.
- Provide up-to-date HCV and HIV-related risk reduction sessions on-site /off-site and refer accordingly to the needs identified by clients and their behavioral/sexual risk assessment/profile.
- Provide services and/or referrals to clients and their families, including: HIV education community forums, food banks, transportation assistance, mental health therapy, and other supports.
- Secure and maintain health screening exams in a HIPPA complaint environment.
- Identify new community partners and data-driven hot spots where HIV rapid testing can be conducted.
- Maintain consistent on-going communication with medical providers to streamline the linkage to medical care process and obtain verbal and written verification of services obtained by clients referred.
