The Resident Care Coordinator oversees all aspects of resident care in Personal Care. The resident care coordinator will be responsible for the overall process of resident assessments, clinical oversight, and support planning (RASP’s) for the neighborhood. The Resident Care Coordinator must demonstrate an awareness that the needs of the residents and family members are first and foremost.
Complete resident assessment support plan’s (RASP’s) and all required documentation in accordance with current regulations on the Personal Care units.
Coordinate and lead the support plan meetings and assure that concerns and treatment plans are addressed, communicated, and implemented.
Make changes to and update the resident support plan based on physician orders, observations, patient response to treatment, and data collected from all available sources.
Communicate with physicians, including primary care physicians and specialists, as well as families and other responsible persons as appropriate.
Monitor the administration of medications to residents according to the facility policy, including appropriate storage, timeliness, and accuracy of dispensing, discarding and documentation.
On the Rockhill campus, serve as the medication administration trainer to conduct initial medication administration training and certification, as well as completing the semiannual observations and documentation reviews. Provide ongoing education and remediation regarding med administration as indicated for both med tech’s and licensed staff.
Participate in interviews and hiring professional and non-licensed staff assigned to the neighborhood, keeping in mind the needs of the whole organization. Assist in orientation and training of staff.
Supervise licensed staff, certified nursing assistants, and homemakers, providing leadership, training, and coaching to the nurse on each shift and ensure that their job is being performed to meet the high standards of the Community and our residents.
Evaluate staff performance as scheduled using accepted tools.
Participate in counseling staff on disciplinary issues including performance, attendance, punctuality, and adherence to their job description and the adherence to policies and procedures.
Carry out an investigation of suspected abuse, neglect, or other resident care issues in a timely and thorough manner and that the details are reported to administration immediately and documented, along with the Director of the Personal Care unit.
Assist in and participate in, the submission and follow-up on all reportable incidents. Monitor reports for trends and potential changes in procedure and policy to prevent future problems. Incorporate recommendations about resident care in the individual resident’s support plan immediately. Assure that information is communicated to all caregivers within a reasonable amount of time.
Make recommendations to the Director of Personal Care during the budgetary process and in clinical decision-making that impact the financial health of the organization.
Oversee clinical operations on the PC units to ensure care and treatment plans are followed per physician’s orders. Ensure licensed staff are completing tasks assigned to them, and that resident care and documentation is complete and accurate.
Monitor clinical staff (med tech’s, CNA’s, homemakers, and licensed staff) to ensure complete and timely communication and follow up with providers, residents, families/ responsible parties.
Minimum - Either RN or LPN
Preferred - RN
Minimum - Three years' experience in a nursing environment including care planning and supervisory experience, if an RN. Five years, if an LPN.
Preferred - Five years' experience in a geriatric setting with assessment responsibilities.
Minimum - Licensed as a Registered Nurse or Licensed Practical Nurse in good standing by the state of Pennsylvania.
Preferred - Certification as a Personal Care Home Administrator.
