Community Health Worker (CHW)
Job Summary
Under the supervision of the Program Manager, the Community Health Worker (CHW) empowers pregnant individuals, parents, caregivers, and families by providing health education, resource navigation, community outreach, and family support throughout Bexar County.
As a trusted member of the community, the CHW helps families identify needs, overcome barriers, and connect to healthcare, social services, and community resources. Community Health Workers at Latched Support are cross-trained to conduct resource navigation appointments, facilitate educational workshops, support outreach events, enroll families into programs, and collaborate with healthcare and community partners to improve maternal, infant, and family health outcomes.
This non-exempt position requires professionalism, compassion, strong communication skills, the ability to build trusting relationships, and a commitment to maintaining HIPAA compliance and client confidentiality.
Essential Duties & Responsibilities
Resource Navigation & Family Support
- Conduct one-on-one resource navigation appointments with pregnant individuals, parents, and families.
- Complete needs assessments and connect families to healthcare, housing, food assistance, transportation, childcare, mental health services, and other community resources.
- Assist families with enrollment into Latched Support programs and community services.
- Conduct follow-up phone calls and outreach to ensure families successfully access recommended resources.
- Provide advocacy, encouragement, and ongoing support while helping families achieve their goals.
Health Education
- Teach evidence-based pregnancy, parenting, newborn care, breastfeeding, child safety, and family wellness workshops after completing organizational training.
- Educate families on healthy behaviors, preventive care, and available community resources.
- Promote maternal, infant, and family health through community education and engagement.
Community Outreach
- Represent Latched Support at community events, health fairs, educational baby showers, outreach events, partner meetings, and other community initiatives.
- Assist with Resource Days, diaper distributions, and family engagement activities.
- Build relationships with healthcare providers, schools, community organizations, and local partners.
- Promote Latched Support services and help connect families to available programs.
Documentation & Collaboration
- Maintain accurate documentation and complete required program records in a timely manner.
- Protect client confidentiality and maintain HIPAA compliance.
- Collaborate with nurses, doulas, lactation consultants, educators, and community partners to ensure coordinated family support.
- Attend staff meetings, trainings, professional development activities, and community partner meetings.
- Perform other duties as assigned to support organizational goals.
QualificationsEducation
- High school diploma or GED required.
- Texas Certified Community Health Worker (CHW) preferred. Candidates willing to obtain certification within an approved timeframe after hire are encouraged to apply.
Experience
- Experience in community health, public health, healthcare, nonprofit services, outreach, resource navigation, case management, social services, or family support preferred.
- Experience working with pregnant individuals, parents, infants, or families is a plus.
- Ability to work independently while collaborating effectively with multidisciplinary teams.
Skills
- Excellent verbal, written, and interpersonal communication skills.
- Strong organizational and time management skills.
- Ability to build trusting relationships with diverse populations.
- Comfortable speaking with individuals and facilitating small group education.
- Basic computer proficiency and experience with electronic documentation.
- Bilingual (English/Spanish) strongly preferred.
- Professional, compassionate, and culturally responsive approach to serving families.
Physical Requirements
- Ability to sit, stand, walk, bend, kneel, and occasionally lift or carry up to 40 pounds of educational materials, diapers, outreach supplies, and event equipment.
- Ability to travel throughout Bexar County for community appointments, workshops, outreach events, and partner meetings.
- Ability to assist with setting up and breaking down community events and educational activities.
- Valid Texas driver's license, reliable transportation, and current automobile insurance required.
Working Conditions
Work is performed in office settings, community centers, healthcare facilities, schools, partner organizations, and outdoor community events. Occasional evening and weekend work may be required to support outreach activities and community programs.
Pay: $18.00 - $20.00 per hour
