Schedule: Full-Time | Four 10-hour shifts (7:00 AM – 5:00 PM)

Join our Care Coordination team and play a vital role in ensuring patients receive compassionate, timely hospice care. As a Care Coordination Specialist, you'll be the first point of contact for patients, families, and clinical staff, coordinating schedules and helping deliver seamless care with exceptional customer service.

Answer inbound calls and provide exceptional customer service to patients, families, and team members.
Schedule and coordinate patient visits based on each patient's individualized plan of care.
Communicate effectively with clinical staff to ensure timely and accurate scheduling.
Maintain accurate scheduling documentation using computer-based scheduling systems.
Participate in Continuous Quality Improvement (CQI) initiatives that support the quality of hospice services.
Assist with onboarding by precepting and supporting newly hired Care Coordination team members as needed.
Collaborate with interdisciplinary team members to provide excellent patient care and service.
Required:
High School Diploma or GED.
Strong computer skills with the ability to learn scheduling software and communicate effectively.
Valid driver's license with a safe driving record.
State minimum automobile insurance coverage.
Ability to travel when needed, including during daytime, nighttime, or inclement weather.
Ability to successfully pass a criminal background check and pre-employment drug screen.
Preferred:
Two or more years of scheduling, dispatching, or care coordination experience.
Experience in healthcare or a customer service-focused environment.
