Physical Therapist | Home Visits – Lakeshore | Part-Time

Atrio Home Care, a trusted partnership between Holland Home and Resthaven, is seeking a compassionate and motivated Physical Therapist to join our Holland/Lakeshore team, serving clients in Ottawa and Allegan Counties.

What You’ll Do:

Provide one-on-one in-home physical therapy services

Conduct evaluations and develop individualized treatment plans

Collaborate with physicians and interdisciplinary team members

Help clients regain mobility, strength, and independence

Document progress and support discharge planning

Educate clients, caregivers and families, on therapy goals and techniques
Schedule & Location:

Territory: Ottawa & Allegan County
-
Days: 3 days/week (approx. 24 hours) – flexible scheduling

Shift: Day shift (8 AM – 5 PM)

Weekend Rotation: Every 4–5 weekends, no on-call

Holidays: 1–2 per year

Pay-for-activity model with weekend/holiday differentials

- Orientation: 6–8 weeks with 1:1 field training ($320/day)
- Mileage reimbursement and additional pay for meetings/in-services

Requirements:

Master's or Doctorate Degree in Physical Therapy (required)

Michigan PT license (required)
-
1+ year of PT experience (home health preferred)

Geriatric experience highly valued

Reliable transportation, valid driver’s license, and auto insurance
