JOB SUMMARY
The Service Coordinator links residents to health and supportive services that will enable the resident to achieve and maintain his/her maximum level of self-sufficiency in independent living.
ESSENTIAL FUNCTIONS
- Performs an “in-home” general needs assessment for each resident on an annual basis.
- Develops and implements a general case management plan for each resident based on the needs assessment.
- Follows-up with residents where appropriate on monthly basis, or on the needs of the individual.
- Maintains an active file for each resident containing all information on social service needs.
- Works cooperatively with the property manager to address related issues.
- Performs resident services surveys of all tenants annually and develops programs responding to unmet needs.
- Works with the resident council to develop activities and programs beneficial to all residents.
- Establishes links with social service providers/volunteer/support groups.
- Handles all reporting responsibilities required by the HUD contract.
JOB REQUIREMENTS AND EXPERIENCE
- High School Diploma required. Bachelor’s degree in social work, Gerontology, Psychology or Counseling preferred.
- Training in aging, elder, or disability services.
- Two years’ experience in social service delivery with senior citizens and non-elderly disabled.
- Demonstrated working knowledge of supportive services for senior citizens and non-elderly disabled in the area.
- Demonstrated ability to advocate, organize, problem solve, and provide results for the elderly and disabled.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
- Physical strength to manipulate light to medium weight materials weighing 10 to 15 pounds on a regular basis.
- Local travel required.
- Periodic attendance at conferences.
