Location: Pueblo, CO
Pay: $38–$45/hour

What You'll Do
- Coordinate and complete accurate, timely MDS assessments in accordance with CMS and state regulations.
- Ensure compliance with Medicare, Medicaid, PDPM, and reimbursement guidelines.
- Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members.
- Develop and maintain comprehensive resident care plans.
- Review documentation to ensure accuracy and appropriate reimbursement.
- Monitor MDS schedules and assessment deadlines.
- Participate in quality assurance initiatives, Triple Check meetings, and survey readiness.
- Provide education and guidance to clinical staff regarding MDS documentation and regulatory requirements.
What You'll Need
- Current Colorado Registered Nurse (RN) license in good standing.
- Previous MDS experience in a Skilled Nursing or Long-Term Care setting required.
- Strong knowledge of PDPM, Medicare, Medicaid, and CMS regulations.
- Excellent assessment, documentation, and organizational skills.
- Ability to work collaboratively with an interdisciplinary team.
- RAC-CT certification is a plus, but not required.
