Job Title: Licensed Practical Nurse (LPN) – Memory Care Position Summary
The Licensed Practical Nurse (LPN) in Memory Care provides compassionate, resident-centered nursing care for individuals living with Alzheimer’s disease and other forms of dementia. This role supports residents’ physical, emotional, and cognitive well-being while promoting dignity, safety, and quality of life. The LPN works closely with Wellness Director, caregivers, families, and healthcare providers to ensure consistent, individualized care.
Key Responsibilities Clinical Care
- Administer medications and treatments according to physician orders and community policies
- Monitor residents’ health status and report changes promptly to the Wellness Director and appropriate provider.
- Perform ongoing assessments including skin integrity, pain, hydration, nutrition, and behavioral status
- Provide wound care and other nursing procedures within scope of practice
- Maintain accurate and timely documentation in the medical record
Memory Care Support
- Provide care using dementia-specific, person-centered approaches
- Recognize and respond appropriately to behavioral expressions related to cognitive impairment
- Use de-escalation and redirection techniques to support residents experiencing distress
- Promote structured routines and therapeutic activities that enhance resident well-being
- Ensure a safe environment for residents at risk for wandering or falls
Care Coordination
- Communicate effectively with families regarding resident condition, changes, and care needs
- Collaborate with caregivers and medication aides to ensure proper care delivery
- Participate in care plan meetings and contribute to individualized service plans
- Coordinate physician visits, lab work, and outside services as needed
Leadership & Team Support
- Supervise and support CNAs/caregivers during shift
- Provide guidance on infection control, safety, and dementia care best practices
- Assist with training and mentoring new staff as needed
- Promote a positive team environment and culture of respect
Compliance & Safety
- Follow all state regulations, infection control guidelines, and community policies
- Ensure proper medication storage, administration, and documentation
- Respond to emergencies and provide first aid or nursing intervention as needed
- Participate in quality improvement and risk management activities
Qualifications
- Current, active LPN license in good standing
- CPR/First Aid certification (or willingness to obtain)
- Minimum 1 year nursing experience preferred; experience in geriatrics or memory care strongly preferred
- Knowledge of dementia care practices and behavioral support techniques

- Frequent standing, walking, bending, and assisting residents with mobility

Schedule
- Part-time / PRN
- Day / Evening / Night shifts
- Weekend and holiday rotation may be required
Job Type: Part-time
Pay: $24.00 - $27.00 per hour
