As a Community Health Worker, you'll serve as a trusted partner and advocate by helping participants navigate healthcare services, coordinate appointments, connect with community resources, and achieve their individual health goals. You'll provide health education and wellness coaching, collaborate with healthcare providers and case managers, accompany participants to medical appointments, and support personalized care plans that promote healthier, more independent living.
You'll also document services accurately to meet Medicaid requirements while ensuring every participant receives compassionate, person-centered support.

- No Community Health Worker certification? No problem! If you're not already certified, LifeScape will provide the required training and cover the cost of certification within your first 90 days of employment (Contingent on acceptance into program and passing fingerprinting requirements)

High school diploma or GED equivalency is required. Must be 18 years of age or older. Valid driver’s license with clean driving record. May require training/testing to drive accessible vehicles. Prior experience working with children or adults with disabilities or challenging behaviors preferred. Successful completion and certification in training provided by agency within specific time allotment required.
Active CHW certification required at hire or must be obtained within the first 90 days as a condition of continued employment. Certification training and associated costs are covered by LifeScape (Contingent upon acceptance into program and passing of background/fingerprint check)
All positions require a clear background check and drug screen upon conditional offer.
