We are seeking a dedicated and detail-oriented Client Care Coordinator to join our team. This vital role involves managing client care, providing counseling, faciliting groups and ensuring access to necessary resources.
Duties
- Develop and implement individualized care plans for clients
- Provide counseling and facilitate therapeutic groups.
- Facilitate access to community resources and services
- Collaborate with multidisciplinary teams to enhance client outcomes
- Document client progress and maintain accurate records.
Requirements
- Bachelor's degree in Social Work, Psychology, or related Human Service field.
- Experience in mental health and social work environments is advantageous.
Pay: $21.00 - $23.00 per hour
