Position Summary:
We are seeking a dedicated and compassionate Targeted Case Manager (TCM) to join our team. The ideal candidate will hold a Bachelor’s Degree in Social Work, possess a current LSW (Licensed Social Worker) credential, and have prior experience in case management. This role is vital in supporting individuals in navigating community resources, coordinating care, and achieving their personal goals related to mental health, substance use, and social well-being.
Key Responsibilities:
- Provide person-centered case management services to individuals with mental health and/or substance use needs
- Conduct comprehensive assessments and develop individualized service plans
- Coordinate access to community services including housing, employment, healthcare, and social supports
- Monitor client progress and adjust service plans as needed
- Maintain regular and timely documentation in compliance with agency and regulatory requirements
- Collaborate with clinical teams, service providers, and support networks
- Advocate for clients’ rights and access to services
- Participate in ongoing training, team meetings, and supervision
Qualifications:
- Bachelor’s Degree in Social Work (BSW) – Required
- Current LSW (Licensed Social Worker) – Required
- Minimum of 1 year of experience in case management – Required
- Knowledge of community resources and social service systems
- Strong organizational, communication, and problem-solving skills
- Ability to work independently and manage a caseload
- Valid driver’s license and reliable transportation (if travel is required)
Preferred Skills:
- Bilingual or multilingual abilities
- Experience with electronic health records (EHR)
- Trauma-informed care or motivational interviewing training
