Position Title: Options Counselor
Reports To: Center Manager
FLSA Status: Non-Exempt
POSITION SUMMARY
The Options Counselor is a member of the Aging and Disability Resource Center team. In this role, they provide information and supported referral, options counseling, application support and follow up as needed and appropriate as part of the ADRC/NH Care path model.
ESSENTIAL JOB DUTI ES, ACCOUNTABILITIES AND RESPONSIBILITIES
- Follows NH Care Collaborative (NHCC), NH DHHS, and ACL protocols for person centered options counseling, data recording, and reporting including, but not limited to accurate documentation of SHIP counseling and training activities
- Maintain SHIP certification and conduct SHIP Counseling appointments as needed
- Performs person centered comprehensive needs assessments, including mental, physical, functional, cultural, financial, environmental, and life goals to determine appropriate referrals.
- Provides clinical eligibility counseling and financial prescreening for State and Federal Programs.
- Develops preliminary care plans, provides long-term support counseling, gathers needed information, and offers limited follow-up based on consumer preferences.
- Follows NHCC, NH DHHS, and ACL protocols for follow up.
- Works collaboratively with ADRC team members and the community partners to provide the most streamlined person-centered access to options and services possible.
- Provides Person Centered Options counseling in the office, hospitals, rehab facilities, nursing homes, or other community settings including individual homes as appropriate
- Provides information, referrals and assistance through telephone, walk-in or email intake as needed.
- Assists with/provides periodic public education sessions and training for team members, community members, community partners, the public and volunteers.
- Participates in continuing education, peer counseling, and training opportunities.
- Assists with program operations as requested by supervisor.

Education and/or Experience
- Bachelor’s Degree or 5 years of equivalent work experience
- Inform USA certified within one year of hire
- SHIP certified within one year of hire
- Options Counseling certified within 6 months of hire.
- Valid Driver’s License & Insurance
- Experience in call management, information and referral, options counseling. Familiarity with preliminary assessment and triage.
- Working knowledge of human service delivery system.
- Ability to use electronic client and resource data base system.
- Facilitation and presentation skills.
- Good interpersonal and computer skills, openness and flexibility in working with diverse groups, and enthusiasm for working collaboratively and with a team.

Pay: $19.00 - $21.00 per hour
