At our Nursing and Rehab Center, we are committed to providing high-quality, compassionate care to our residents. We are currently seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Skilled Care Coordinator.
Days: Tuesday and Wednesday and every other weekend/ every weekend if desired.
Hours: 11:00 AM-7:00 PM
This role is key in supporting residents during their transition from hospital to home or assisted living, ensuring a smooth and safe recovery process.
Job Description: As a Transitional Nurse, you will work closely with patients, families, and the healthcare team to provide transitional care that supports patients’ recovery, reduces hospital readmissions, and promotes a seamless transition between care settings. You will assess the health needs of each resident, educate families, and collaborate with interdisciplinary teams to create and implement individualized care plans.
Responsibilities:
- Conduct comprehensive assessments of new admissions and residents transitioning from hospital settings.
- Develop personalized care plans that align with residents' physical, emotional, and medical needs.
- Administer medications, treatments, and procedures as ordered by physicians.
- Monitor and document residents' health status, including vital signs, symptoms, and response to treatments.
- Provide education and support to residents and their families on managing health conditions and following care plans.
- Act as a liaison between hospital discharge planners, physicians, families, and staff to ensure continuity of care.
- Evaluate and adjust care plans based on patient progress and changing needs.
- Work in collaboration with the multidisciplinary team to improve care quality and outcomes.
- Assist with discharge planning, including ensuring all necessary follow-up care and resources are in place.
Qualifications:
- Valid Registered Nurse (RN) license in GA.
- Minimum of 3years of nursing experience, preferably in a long-term care or transitional care setting.
- Strong clinical assessment skills and knowledge of medical conditions and treatments.
- Excellent communication skills, both written and verbal, with the ability to educate patients and families effectively.
- Ability to work well in a team-oriented environment.
- Compassionate, empathetic, and dedicated to improving the quality of life for patients.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications preferred.
