Physical Therapist (Home Health) - Wichita, KS
Legacy Home Healthcare is seeking a compassionate and skilled Physical Therapist (PT) to join our growing Home Health team. In this role, you will provide individualized therapy services to patients in their homes, helping them restore mobility, reduce pain, and achieve their highest level of functional independence.

What You'll Do
As a Home Health Physical Therapist, you will:
- Perform comprehensive patient evaluations and ongoing assessments
- Develop individualized treatment plans in collaboration with physicians and the interdisciplinary care team
- Provide therapeutic interventions to improve strength, balance, mobility, and functional independence
- Educate patients and family members on treatment plans, exercises, and safe mobility techniques
- Train patients in the use of assistive devices such as walkers, canes, wheelchairs, braces, and prosthetics
- Monitor patient progress and adjust treatment plans as appropriate
- Document evaluations, treatments, progress notes, and discharge summaries in a timely manner
- Communicate patient status updates and recommendations to physicians and care team members
- Supervise Physical Therapy Assistants and Home Health Aides in accordance with agency policies and regulatory requirements
- Participate in case conferences, staff meetings, and care coordination activities
What We're Looking For
Required Qualifications
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of practice
- Current CPR certification
- Valid driver's license, reliable transportation, and proof of insurance
- Ability to travel to patient homes within the assigned service area
- Strong communication and documentation skills
Preferred Qualifications
- One or more years of experience in home health, rehabilitation, skilled nursing, or acute care
- Experience working with geriatric and medically complex patients
- Knowledge of Medicare home health regulations and documentation requirements
Physical Requirements
- Ability to perform patient assessments and provide hands-on therapeutic treatment
- Frequent standing, walking, bending, reaching, and lifting
- Ability to lift and carry equipment up to 30 pounds
- Ability to assist with patient transfers and mobility activities
