Penn Home Health Care is seeking a Speech-Language Pathologist (SLP) to provide skilled speech therapy services in the home setting.
Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type:
Per Visit / PRN
Responsibilities:
- Perform speech therapy evaluations and reassessments.
- Develop individualized treatment plans.
- Address communication, cognitive, and swallowing disorders.
- Educate patients and caregivers.
- Collaborate with physicians and interdisciplinary team members.
- Complete timely and accurate documentation.
Qualifications:
- Current Pennsylvania SLP license.
- Certificate of Clinical Competence (CCC-SLP) preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Home health experience preferred.
