Responsibilities:
Provide social work services to residents, including assessments, care planning, and interventions to meet their emotional, psychological, and social needs
Collaborate with interdisciplinary teams to develop and implement individualized plans of care
Conduct psychosocial assessments, identify resident strengths and needs, and develop strategies to address them
Provide counseling, support, and education to residents and their families to help them cope with changes and challenges
Facilitate communication and problem-solving between residents, families, and healthcare teams
Maintain accurate and timely records of resident services and progress
Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements:
Licensed Social Worker (LSW) in the state of Texas or eligible for licensure
At least 0 years of experience in social work, gerontology, or a related field
Strong communication, interpersonal, and problem-solving skills
Ability to work effectively in a fast-paced environment and prioritize multiple tasks
