With offices across North Carolina, we’re currently seeking a skilled Licensed Practical Nurse to join our team. This role may require travel to our other offices within the Coastal region. This position is responsible for assessing and triaging patient concerns and conditions to establish the appropriate level of care required to safely meet the patient's medical needs. The LPN will utilize all resources and guidelines to effectively assess, prioritize and escalate calls when necessary to the appropriate provider. This role is performed from an office setting and must always maintain patient confidentiality including appropriate use of cell phone, email, text messaging, patient records, and EMR.

Qualifications and Experience
-
Current North Carolina Practical Nurse license required. May also include a compact state license.

Graduate of an accredited school of Nursing.

Minimum two years of clinical nursing experience.
-
Ability to act with integrity, professionalism, and confidentiality.
-
Experience using electronic medical records systems.
-
Excellent communication and interpersonal skills.
-
Excellent organizational skills and attention to detail.
Responsibilities include, but are not limited to, the following
-
Conducts patient assessments by asking highly specific questions to accurately determine their concern and refer them to appropriate provider care team.

Obtains and documents the patient medical history and all pertinent information related to prior treatment.

Establishes appropriate level of care required to safely meet the patient's medical needs (emergent, urgent, or non-urgent status).

Initiates diagnostic and therapeutic measures as indicated by clinical protocol.

Provides patients with detailed instructions, outlined in the clinical protocol as well as approved reference material delegated by the provider.

Maintains an open line of professional communication with the physician care team regarding patient needs.

Ensures documentation in the medical record accurately reflects patient communication and plan of care.
Other

Follows all applicable department and practice policies and procedures.

Maintains patient confidentiality and complies with HIPAA privacy and confidentiality guidelines established by the practice.

Maintains detailed knowledge of practice management and other computer software as it relates to job function.

Performs other duties as required and assigned by the Manager or Team Lead.

Attends all regular meetings.
