Position Summary
The Registered Dietitian plays a key role in the interdisciplinary care team by providing expert renal nutrition counseling, developing individualized care plans, and supporting dialysis patients in achieving their nutritional and health goals. Working closely with physicians, nurses, social workers, and other healthcare professionals, this role helps drive positive patient outcomes while ensuring compliance with CMS, ESRD, and facility standards.
Essential Responsibilities
- Serve as the primary nutrition expert for dialysis patients.
- Conduct comprehensive nutritional assessments and develop individualized care plans.
- Monitor laboratory values and adjust nutrition interventions as needed.
- Provide patient education regarding renal diets, fluid management, phosphorus, potassium, protein intake, and overall nutrition goals.
- Monitor and document patient progress in the electronic medical record (EMR).
- Participate actively in interdisciplinary care conferences and treatment planning.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to optimize patient care.
- Maintain accurate and timely documentation in accordance with CMS, ESRD, and facility requirements.
- Advocate for patients while maintaining professional accountability and clinical standards.
- Ensure compliance with all applicable dialysis protocols, policies, and regulatory requirements.
- Complete initial assessments, monthly progress notes, and ongoing nutrition evaluations.
Qualifications
- Current registration with the Commission on Dietetic Registration (CDR).
- Strong knowledge of Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and dialysis nutrition management (preferred)
- Excellent communication, organizational, and documentation skills.
- Ability to work independently and manage a patient caseload effectively.
- Experience with electronic medical records (EMR) preferred.
- Minimum one (1) year of renal or dialysis experience preferred.
