Position Title: Registered Nurse (RN)

Reports To: Director of Patient Services (DPS) / Clinical Supervisor

We are seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our Agency. The RN is responsible for conducting client assessments, developing and updating individualized plans of care, supervising Home Health Aides and Personal Care Aides, ensuring compliance with New York State Department of Health regulations, and promoting high-quality, patient-centered care in the home setting. These responsibilities are consistent with common RN roles in New York home care.
- Conduct comprehensive initial nursing assessments and determine client eligibility for home care services.
- Develop, implement, and revise individualized Plans of Care based on client needs.
- Perform required reassessments and supervisory visits in accordance with NYS DOH
- Supervise and evaluate HHAs and PCAs to ensure safe, effective, and quality care.
- Educate clients, families, and caregivers regarding health conditions, medications, safety, and the Plan of Care.
- Coordinate care with physicians, Managed Long-Term Care plans, case managers, therapists, and other healthcare providers.
- Monitor client progress and promptly report changes in condition to the physician and clinical leadership.
- Maintain accurate, timely, and complete clinical documentation.
- Ensure compliance with agency policies, HIPAA, infection control, and regulatory standards.
- Participate in quality assurance, performance improvement, and staff education activities.
- Provide clinical guidance and support to field staff as needed.
- Participate in on-call rotation when assigned.
- Perform other duties as assigned.
Required:
- Current New York State Registered Nurse (RN) license in good standing.
- Minimum of one (1) year nursing experience; home care experience preferred.
- Valid driver's license and reliable transportation.
- Excellent assessment, communication, and documentation skills.
- Ability to work independently and manage multiple priorities.
- Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and patient education abilities.
- Ability to effectively supervise Home Health Aides and Personal Care Aides.
- Knowledge of infection prevention, safety practices, and regulatory compliance.
- Strong organizational and time-management skills.
- Proficiency with electronic documentation systems and Microsoft Office.
