Reporting to the Manager of Case Management, the Clinical Case Manager Assistant (LPN) partners with the Case Manager to support safe, timely, and effective patient discharges. This role has primary responsibility for coordinating clinically driven discharge services, post‑discharge appointments, and licensed clinical supply and equipment needs.
The Clinical Case Manager Assistant applies practical nursing expertise to ensure continuity of care across the inpatient stay, discharge transition, and post‑discharge period. Success in this role requires strong organization, effective communication, clinical judgment, and problem‑solving skills.
Support discharge planning processes to ensure smooth transitions
Coordinate and schedule follow‑up medical appointments, including obtaining and documenting verbal orders when appropriate.
Collaborate with providers to manage appointments occurring during the rehabilitation stay (maintain, reschedule, or convert to virtual visits as appropriate).
Ensure discharge tasks are completed 24–48 hours prior to planned discharge in collaboration with the interdisciplinary team.
Clinical Equipment, Supplies, and Services (LPN Scope)
Coordinate with the Case Manager to manage clinically complex discharge needs, including oxygen, tracheostomy supplies, IV antibiotics, tube feedings, wound care, and catheter supplies.
Review discharge orders for accuracy and collaborate with therapy, respiratory, nursing, pharmacy, and vendors to ensure timely ordering and delivery of required clinical equipment and supplies.
Confirm delivery timelines, resolve barriers, and communicate equipment needs, delivery expectations, and applicable co‑pay responsibilities to patients and families prior to discharge.
Communication & Documentation
Minimum one year of experience in a healthcare setting
Experience with discharge planning, post‑acute coordination, or DME management
Strong verbal and written communication
Excellent organization and time‑management skills
Effective problem‑solving and critical thinking
Computer and electronic documentation proficient
Ability to manage multiple priorities in a fast‑paced clinical environment
