Alpine Rehabilitation and Nursing Center in Xenia, Ohio is hiring a full-time Occupational Therapist (OT). We're a 99-bed skilled nursing and rehabilitation center on Office Park Drive in Greene County — minutes from Beavercreek, Fairborn, Yellow Springs, and about 15 minutes from Dayton. We're looking for an Occupational Therapist who wants a manageable caseload, a therapy team that actually works together, and a building where staff know every resident by name.
This Occupational Therapist position is full time and on-site. Our residents come to us for short-term rehabilitation after surgery, illness, or a hospital stay, and we also care for long-term skilled nursing residents, a secured memory care unit, and respite stays — so the Occupational Therapist caseload here is varied and never boring. You'll work side by side with our physical therapy (PT) and speech-language pathology (SLP) staff, nursing, and the interdisciplinary team on individualized, person-centered plans of care. Prior experience as an Occupational Therapist in a skilled nursing facility (SNF) or long-term care (LTC) setting is strongly preferred.

What You'll Do
Evaluate new admissions and develop individualized occupational therapy treatment plans and functional goals
Provide skilled occupational therapy treatment to short-term rehab and long-term care residents, including activities of daily living (ADL) retraining, upper extremity strengthening, functional mobility, cognition, and safety
Recommend and train residents on adaptive equipment, durable medical equipment, and compensatory techniques
Complete documentation, daily notes, progress notes, and discharge summaries accurately and on time, in line with Medicare and Medicaid requirements
Participate in care plan meetings and communicate with nursing, physical therapy (PT), speech-language pathology (SLP), and physicians
Supervise Certified Occupational Therapy Assistants (COTAs) as assigned
Educate residents, families, and caregivers to support safe discharges home
Qualifications
Degree in Occupational Therapy (bachelor's, Master of Occupational Therapy / MOT, or Doctor of Occupational Therapy / OTD) from an ACOTE (Accreditation Council for Occupational Therapy Education) accredited program
Current Ohio Occupational Therapist (OT) license — or license eligibility in the State of Ohio
NBCOT (National Board for Certification in Occupational Therapy) certification / Occupational Therapist Registered (OTR) status
Experience as an Occupational Therapist in a skilled nursing facility (SNF), nursing home, or long-term care (LTC) setting is preferred
Comfortable working with a geriatric population, including residents with dementia and Alzheimer's disease
Solid documentation skills and familiarity with Medicare Part A and Part B guidelines
Team player who communicates well with nursing and the rest of the rehab department
