Help Bridge the Gap Between Hospital and Home.
We're looking for a compassionate, organized behavioral health professional to serve as our State Psychiatric Hospital (SPH) Gatekeeper. In this role, you'll coordinate care for individuals receiving treatment in Indiana State Psychiatatric Hospitals, helping ensure they receive the right services, experience smooth transitions back into the community, and have the support they need to succeed.
What You'll Do
- Coordinate referrals, admissions, and discharges for State Psychiatric Hospital clients.
- Partner with hospitals, courts, correctional facilities, and community providers to coordinate care.
- Conduct hospital visits and follow-up assessments to monitor progress and prepare for discharge.
- Connect clients with housing, benefits, transportation, medication management, and other community supports.
- Manage involuntary commitment processes and attend court hearings as needed.
- Maintain accurate documentation and ensure compliance with state and federal regulations.
- Participate in multidisciplinary treatment planning and collaborate with community partners to improve client outcomes.
What You'll Bring
Required
- Bachelor's degree in a human services-related field.
- Five (5) years of relevant behavioral health or social services experience.
- Valid Indiana driver's license, reliable transportation, and current auto insurance.
Preferred
- Master's degree in Social Work, Counseling, Psychology, or a related behavioral health field.
- Two (2) years of post-graduate professional experience.

Pay: $47,382.40 - $54,489.76 per year
