Job Overview:
We are seeking a dedicated and experienced LPN Charge Nurse to join our healthcare team. The LPN Charge Nurse will play a critical role in overseeing patient care and ensuring high standards of nursing practice. This position requires strong leadership skills, clinical expertise, and a commitment to providing & ensuring exceptional patient care.
Job Duties:
- Oversees and promotes residents’ overall health and wellness through regular visits with each resident, resident assessments, and ongoing communication with the resident care team including DON, families, physicians, Home Health, and Hospice providers.
- Provide hands-on supervision of caregiving staff, ensure provision of optimal resident care, and uphold company policies.
- Provide direct assistance with activities of daily living and support to geriatric, memory care residents by providing specialized care tailored to meet their individual needs.
- Provides medication administration of scheduled and PRN medications to residents in accordance with doctor’s orders, state regulations, and company policies and procedures.
- Order medications, check in medications, properly store medications and ensure proper documentation is completed.
- Maintain a neat, clean, organized medication cart and restock cart as needed.
- Ensure the residents’ rights to safe, prompt, and confidential treatment.
- Ensure that all infection control guidelines are met.
- Maintain residents’ care charts and enter Nurse Notes as needed.
- Answer and respond immediately to resident call lights, emergency lights, bed/chair alarms, door alarms, as well as facility phone calls.
- Provide a thorough shift report to oncoming nursing staff and complete walking round together to ensure comprehensive shift handoff and continuity of resident care.
Qualifications:
- Current nursing license with relevant certifications such as CPR training.
- Experience working with geriatric and memory care populations is highly desirable.
- Excellent communication skills, both verbal and written, with the ability to lead a team effectively.
- Strong critical thinking skills and the ability to make sound decisions under pressure.
Schedule:
- Full time Night Shift position
- Monday-Thursday 10:00pm-6:00am
- 8 hour shifts
- 3 days off (Friday, Saturday, & Sunday)
Physical Demands of Position:
- Stand/walk up to 6 hours a day
- Sit up to 2 hours a day
- Frequently support up to 70 pounds
- Occasionally lift/carry up to 50 pounds
- Frequently kneel, bend, and reach
- Secure proper assistance for transferring and lifting of residents as needed
