Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient health status, recording vital signs and noting any changes in condition
- Administer medications and perform basic medical procedures such as catheter care and wound care under supervision
- Support patients with memory care needs, including Alzheimer’s and dementia care, ensuring a safe environment
- Utilize Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans accurately
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Follow home healthcare regulations and HIPAA guidelines to maintain patient privacy and safety standards
- Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment
- Operate medical equipment like Hoyer lifts or other assistive devices safely for patient transfers
- Collaborate with nursing staff and other healthcare professionals to deliver comprehensive patient care
Skills
- Proven experience working with individuals with disabilities or in assisted living environments
- Strong knowledge of medical terminology, medication administration, and vital signs monitoring
- Familiarity with EMR/EHR systems for accurate documentation of patient care activities
- Expertise in dementia, Alzheimer’s, hospice, and end-of-life care practices
- Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or wheelchairs
- Excellent caregiving skills with a compassionate approach tailored to geriatric patients and those with developmental disabilities
- Knowledge of home & community care regulations and best practices in home healthcare settings
- Effective communication skills for working with patients, families, and healthcare teams
- Basic life support (BLS), CPR certification, first aid training, and knowledge of hospice care protocols
