JOB SUMMARY:
The hospice nurse practitioner works in collaboration with Hospice of Spokane staff to provide comprehensive, quality hospice and palliative care services to patients under the direction of the Hospice Medical Director.
Responsibilities
- Perform patient visits wherever the patient may call home.
- Provide face-to-face encounter documentation to include narrative explanation of clinical finding that support a life expectancy of six months or less; attestation form that includes date of encounter and of the information collected being provided to the Hospice Medical Director.
- Provide medical expertise in pain, symptom management, and care at the end-of-life to patients, family members and staff.
- Prescribes medications, working hand-in hand with patient’s primary care physician and the HOS Medical Director
- Orders and interprets laboratory and radiology tests when necessary.
- Ensures that care being provided is based on established hospice and palliative care guidelines
- Provide on-call coverage.
- Complete death certificates.
- Perform other duties as assigned.
Current RN license and Nurse Practitioner license to practice in the state of WA.
2-4 years clinical nursing experience; oncology, hospice, geriatrics, extended care facility and/or intensive care.
Minimum of two years of Hospice experience preferred.
Valid Drug Enforcement Agency registration
Excellent organizational, interpersonal and communication skills; ability to work effectively with a ride range of constituencies in a diverse community.
Computer literacy in Microsoft Office applications and EMR/EHR.
Health Care Providers (CPR and AED) certification required.
A valid driver’s license, acceptable driving record and personal vehicle for on-the-job transportation with evidence of auto insurance coverage as required by the State of Washington.
Pay: $125,000.00 - $165,000.00 per year
