Personal Home Health & Hospice is seeking a dedicated and compassionate individual to provide care to patients in the home setting throughout Wayne county. Qualified candidate will possess a Pennsylvania Certified Nurse Assistant Certification in good standing and will have reliable transportation to travel between clients.
Compensation is $20 per visit and mileage reimbursement of $0.54 per mile.
Job Duties include:
- Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, and feeding.
- Provide safe ambulation, transfers, and positioning, including use of assistive devices.
- Offer skin care, simple wound care support (non-sterile), and proactive pressure injury prevention.
- Support patient mobility and exercise programs as directed by clinical staff
- Maintain a clean, safe, and comfortable home environment.
- Assist with light housekeeping, including laundry, linen changes, and tidying patient areas.
- Prepare simple meals following dietary guidelines.
Communication & Documentation
- Document all care provided in compliance with agency and regulatory standards.
- Immediately report changes in patient condition to the RN case manager.
- Maintain clear, professional communication with patients, families, and the care team.
Compliance & Professionalism
- Follow all agency policies, infection control standards, and state/federal regulations.
- Uphold patient rights, privacy, and dignity at all times.
- Participate in required in‑service training and performance evaluations.
Qualifications Required
- High school diploma or equivalent.
- Pennsylvania Certified Nurse Assistant Certification in good standing
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Ability to pass state and federal background checks, drug screening, and health screenings (including TB testing).
- Ability to lift, pull, and transfer up to 50 lbs.
- 6+ months experience as a CNA (home health, hospice, or facility‑based) preferred.
- Strong interpersonal skills and comfortable working independently in patient homes.
Work Environment
- Care is provided in patient homes throughout Wayne county.
- Travel required between patient visits.
- Occasional exposure to pets, household environments, and varying weather conditions.
