MDS Coordinator

MDS Coordinator Responsibilities:
Conduct thorough and accurate Minimum Data Set (MDS) assessments for our residents, ensuring compliance with federal and state regulations
Collaborate with interdisciplinary team members to develop and implement individualized care plans that meet each resident's unique needs
Maintain accurate and up-to-date records of assessments, care plans, and changes in resident condition
Provide education and training to staff members on MDS assessment and care plan development
Contribute to the development and implementation of facility-wide quality improvement initiatives
Participate in quality assurance and performance improvement activities to ensure optimal resident care and outcomes
Maintain a strong competence in MDS assessment and care planning principles and regulations
