Job Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic home health team. In this vital role, you will provide high-quality nursing care directly in patients' homes, ensuring their comfort, safety, and well-being. Your expertise will support individuals across various stages of health, including those with complex medical needs such as dementia, Alzheimer's, and chronic illnesses. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
- Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards.
- Manage wound care, catheterization, tube feeding, ventilator support, and other specialized nursing procedures with precision.
- Monitor patient progress and document clinical findings accurately using electronic health record (EHR) systems such as Epic or eClinicalWorks.
- Educate patients and their families on disease management, medication usage, and lifestyle modifications to promote health and independence.
- Coordinate with interdisciplinary teams for case management, triage, and urgent care needs to ensure seamless patient care delivery.
- Maintain compliance with HIPAA regulations and long-term care standards while ensuring confidentiality of all medical records.
Qualifications
- Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN).
- Proven experience working in home health settings or skilled nursing facilities preferred.
- Strong knowledge of medical terminology, physiology, and patient assessment techniques.
- Experience with EMR/EHR systems such as Epic or Cerner is highly desirable.
- Competency in critical care procedures including IV infusion, catheter care, airway management, and pain management.
- Familiarity with diverse patient populations including geriatrics, pediatrics, individuals with developmental disabilities or dementia.
- Ability to work independently with excellent caregiver communication skills and a compassionate approach to patient care.

Pay: $45.00 - $65.00 per hour
Expected hours: 5.0 – 20.0 per week
