Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including long-term care, assisted living, and acute care facilities. You will be responsible for supporting patients with diverse medical needs, ensuring their comfort and safety while adhering to best practices in nursing.
Responsibilities
- Deliver direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to evaluate patient conditions.
- Assist in the development and implementation of individualized care plans.
- Provide support for patients with developmental disabilities, Alzheimer's, dementia, and other memory-related conditions.
- Perform IV infusions, catheterization, and wound care as needed.
- Document patient information accurately in electronic health records (EHR) systems.
- Collaborate with healthcare teams to ensure comprehensive patient care.
- Educate patients and their families on health management and disease prevention strategies.
- Maintain compliance with HIPAA regulations and infection control protocols.
Skills
The successful candidate should possess a wide range of skills including but not limited to:
- Proficiency in EMR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Experience in critical care settings such as ICU or emergency medicine.
- Knowledge of medical terminology, anatomy, physiology, and basic life support techniques.
- Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation.
- Competence in performing phlebotomy, venipuncture, specimen collection/processing, and administering injections.
- Strong patient assessment skills with the ability to recognize changes in patient conditions.
- Experience working with geriatrics and providing senior care services.
- Ability to manage acute pain effectively and provide hospice or palliative care when necessary.
