Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories.
- Administer medications, injections, IV infusions, and treatments such as botulinum toxin therapy or catheterizations with precision and adherence to safety protocols.
- Manage airway support and ventilator care for patients requiring respiratory assistance.
- Document all patient interactions accurately using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks.
- Coordinate care plans with multidisciplinary teams, including case management and discharge planning to ensure seamless patient transitions.
- Perform specimen collection and processing, phlebotomy, suturing, and sterile procedures following aseptic techniques.
- Provide specialized care in areas such as dementia/memory care, hospice support, infectious disease management, or trauma response in Level I/II trauma centers.
- Support patients with complex needs such as tube feeding, IV insertion and maintenance, catheter care, pain management for acute or chronic conditions, and wound care.
- Educate patients and families on health coaching topics like medication administration, nutrition, injury prevention, and post-acute recovery strategies.
- Ensure compliance with HIPAA regulations and maintain meticulous medical documentation to uphold privacy standards.
Requirements
- Valid LPN or RN license in good standing; additional certifications such as Basic Life Support (BLS), ACLS (Advanced Cardiovascular Life Support), or PALS (Pediatric Advanced Life Support) are preferred.
- Proven experience in diverse clinical environments including hospital medicine, ICU/CCU/PCU units, emergency rooms, outpatient clinics or home & community care settings.
- Critical care experience in settings like trauma centers or Level I/II trauma units is highly desirable.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures and diagnoses is a plus.
- Strong skills in patient assessment, physiology knowledge, infection control practices and medication administration.
- Experience working with individuals with disabilities or developmental delays; dementia or Alzheimer’s care experience is advantageous.
- Ability to perform venipuncture, phlebotomy, sterile procedures including suturing and spinal taps when necessary.
