Position Summary
The Hospice Quality of Life Enrichment Specialist is responsible for creating meaningful, individualized experiences that enhance the emotional, social, cognitive, and spiritual well-being of hospice patients. Working collaboratively with our Social Workers, Nurses, Chaplains, and interdisciplinary team, you will help patients create joyful moments, foster meaningful connections, and maintain dignity throughout their hospice journey.
Responsibilities
- Plan and facilitate individualized life enrichment activities based on each patient's interests, abilities, and preferences.
- Engage patients through music, arts and crafts, storytelling, reminiscence therapy, games, reading, and other meaningful activities.
- Provide companionship and emotional support through one-on-one visits.
- Offer comfort-focused self-care experiences, such as painting nails or assisting with simple grooming activities that promote dignity and self-esteem.
- Adapt activities for patients living with dementia, Alzheimer's disease, developmental disabilities, or other cognitive or physical limitations.
- Partner with Social Workers and the interdisciplinary hospice team to develop personalized engagement plans.
- Encourage family involvement in creating meaningful memories whenever appropriate.
- Observe patient responses and communicate changes or concerns to the care team.
- Document patient interactions in accordance with company policies.
- Maintain a compassionate, respectful, and patient-centered approach in every interaction.
Qualifications Required
- High school diploma or equivalent.
- At least one (1) year of experience in healthcare, hospice, home health, assisted living, memory care, skilled nursing, or a related setting.
- Experience working with individuals living with dementia or Alzheimer's disease.
- Passion for improving quality of life through meaningful engagement and companionship.
- Strong communication and interpersonal skills.
- Ability to work independently while collaborating with an interdisciplinary healthcare team.
- Reliable transportation and a valid driver's license.

- Hospice or Palliative Care

- Recreational Therapy or Therapeutic Recreation
- Activities Coordination or Life Enrichment
- Social Work
- Child Development
- Developmental Disabilities Services
- Behavioral Health
- Adult Day Programs
- Senior Living Communities
