Position Summary
VidaWell Care is seeking a licensed Occupational Therapist to provide skilled OT services to home health patients throughout Miami-Dade County. The ideal candidate is patient-centered, detail-oriented, and comfortable working independently while collaborating closely with our multidisciplinary care team.
Responsibilities
- Conduct initial OT evaluations and develop individualized treatment plans (OASIS documentation experience preferred)
- Provide in-home occupational therapy services to improve patients' functional independence with ADLs/IADLs
- Assess home safety and recommend adaptive equipment or modifications
- Collaborate with RNs, PTs, and other care team members to coordinate patient care
- Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and AHCA regulations
- Communicate effectively with patients, families, and referring physicians
Qualifications
- Current Florida OT license in good standing
- Home health experience strongly preferred; OASIS certification a plus
- Valid driver's license, reliable transportation, and active auto insurance
- Strong communication skills; bilingual (English/Spanish) a plus
- Ability to work independently and manage a flexible visit schedule
- CPR certification
