Evaluate residents and develop individualized speech therapy plans of care.
Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
Perform dysphagia evaluations and implement appropriate swallowing interventions.
Reassess patient progress and adjust treatment plans as needed.
Complete timely and accurate documentation using the electronic medical record (EMR).
Educate residents, caregivers, and facility staff on communication strategies, swallowing precautions, and home exercise programs.
Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient-centered care.
Maintain a productive clinical caseload while delivering compassionate, evidence-based therapy services.
Current State Speech-Language Pathologist (SLP) License
Current Certificate of Clinical Competence (CCC-SLP) preferred (or eligibility if applicable)
Current CPR/BLS Certification
Valid driver's license, reliable transportation, and auto insurance
Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
