Responsibilities
- Assist residents or patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs such as blood pressure, pulse, and temperature regularly
- Provide medication reminders and assist with medication administration following care plans and HIPAA regulations
- Support patients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement
- Prepare nutritious meals tailored to individual dietary needs, ensuring proper food safety and cleanliness standards
- Maintain detailed documentation of patient observations, care activities, and progress notes in compliance with healthcare protocols
- Utilize equipment such as Hoyer lifts or resident’s <PERSON> to safely transfer individuals requiring heavy lifting or assistance with mobility
- Conduct routine cleaning of living spaces to uphold hygiene standards and promote a healthy environment
- Collaborate with healthcare professionals to develop and implement personalized care plans that meet each individual’s unique needs
- Engage in patient monitoring to observe changes in condition or behavior, reporting concerns promptly to supervisors or medical staff
Experience
- Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly preferred
- Knowledge of working with individuals with developmental disabilities, autism, dementia, or memory impairments is advantageous
- Certification in first aid and CPR is required; additional certifications such as HIPAA compliance are a plus
- Demonstrated ability to perform patient observation and monitor vital signs accurately
- Experience assisting with medication administration and implementing care plans effectively
- Familiarity with hospice care principles and home health practices enhances your ability to provide comprehensive support
- Ability to perform heavy lifting safely using equipment like Hoyer lifts when necessary
- Strong interpersonal skills for working compassionately with seniors, individuals with disabilities, or those requiring specialized memory care
