Responsibilities
- Deliver comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following protocols.
- Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or memory impairments.
- Manage acute pain and facilitate pain management strategies tailored to individual needs.
- Document all patient interactions thoroughly using Point Click Care while maintaining strict compliance with HIPAA and confidentiality policies.
- Collaborate effectively with multidisciplinary teams to for optimal patient outcomes.
Experience
- Valid state licensure as a Licensed Practical Nurse (LPN).
- Experience with long-term care facilities, skilled nursing facilities, or assisted living communities enhances your application.
- Experience working with individuals with disabilities or complex chronic conditions or hospice care is a plus.

Pay: $24.00 - $30.00 per hour
